PGY1 Anesthesia/Ultrasound Rotation

This is a week rotation in Anesthesia as well as emergency point-of-care ultrasound (POCUS), with one week of PTO. Before the start of your rotation, please inform Dr. Ho-Gotshall of your PTO days.

In addition to the Anesthesia rotation goals and objectives, this educational experience is in place to provide you with the core knowledge base as it pertains to POCUS for bedside image acquisition and interpretation. This month will serve as a framework to allow you to use bedside ultrasound in your daily practice of Emergency Medicine

During the block, residents will attend EUS didactics on basic & advanced emergency EUS applications. The training sessions cover fundamental ultrasound physics, as well as diagnostic and procedural emergency ultrasound examinations and interpretations.

During orientation & the PGY1 rotation, a combination of didactics, experiential learning and proficiency assessments will be employed. The following clinical ultrasound application topics will be covered:

  • Trauma/EFAST

  • Intrauterine Pregnancy (transabdominal and transvaginal)

  • AAA

  • Cardiac/Volume Assessment

  • Biliary

  • Renal/Urinary Tract

  • DVT

  • Soft Tissue/Musculoskeletal

  • Thoracic/Airway

  • Ocular

  • Procedural Guidance

  • Bowel

CLINICAL EXPERIENCE AND SCHEDULE

  1. Supervised scanning shifts - please refer to the Google calendar (linked here and under Rotation Logistics column above) for dates/times to meet faculty in the ED for bedside scanning and teaching. An Ultrasound faculty member will meet with you on the first day of the rotation for orientation and introductory scanning shift. The goal is to become familiar with our POCUS equipment, workflows and our middleware (QPathE).  As an adult learner, the rest of your time is independent and you must meet both Anesthesia and Ultrasound rotation requirements.

  2. Independent scanning shifts - during times when there is no scheduled supervised scanning, the resident should be scanning independently in the ED. Patients scanned independently should have a confirmatory study as a gold standard (e.g. CT abdomen, CXR, confirmatory US performed by radiology). On days with unassigned activities, try and join POCUS faculty on clinical shifts. POCUS faculty include: Dr. Wilson, Dr. Pope, Dr. Ho-Gotshall.

  3. Radiology tech scanning shifts - Tuesdays at Grand Strand & Wednesdays at South Strand from 4:30 - 7:00 p. Please refer to the POCUS Google Calendar for the Ultrasound Technician you will be scanning with. We ask that you reach out to them the day of to confirm meeting time and place. Their numbers should be listed on the Google Calendar. You are responsible for communicating with the ultrasound technicians if you are not available to scan with him for any reason, but you are otherwise expected to be there. They are RDMS and RVT certified and can serve as your guide for abdominal and DVT studies.

  4. During your anesthesia/ultrasound block, you are required to join the weekly education/QA meetings - these take place every Thursday at 9am the GME/Ocean View Building in Classroom 5. Reference the Google Calendar each week to ensure there are no conferences or special events that impact the timing or location. Please join in the GME Building after you’ve finished obtaining your airways for the day, you are welcome to come and go in order to maximize your airway opportunities.

  5. You still have access to the Core ultrasound lectures, please reference them if you are looking for more didactic material on a particular ultrasound application.

Requirements

1) CLINICAL 

  • Perform at least 75 high quality and correctly interpreted ultrasound studies and complete appropriate reports in QpathE.  Required views are listed on this website and on your laminated cards you were given at Boot Camp.

  • Attend all THURSDAY education/QA meetings while on rotation.
